CSW says that the Burma Army has been conducting a military offensive against the Kachin since June 2011, breaking a ceasefire that had lasted for 17 years. Over 100,000 civilians have been internally displaced since the war began, and the Burma Army has been accused of grave human rights violations, including rape, torture, destruction of villages, looting and desecration of churches, and killing civilians. The offensive has intensified severely in recent days.

NEW YORK (Reuters Health) - Getting doctors together to discuss the best treatments for cancer patients in U.S. Veterans Affairs hospitals was only linked to a minor improvement in care in a large new study.
Analyzing the records of 138 VA medical centers, researchers found that the presence of a so-called tumor board - a group of cancer treatment experts - only affected seven out of 27 measures of quality and processes in patient care, and not always for the better.
"This does not support the hypothesis that tumor boards are doing a lot to improve care," said Dr. Nancy Keating from Boston's Harvard Medical School and Brigham and Women's Hospital, the study's senior author.
Tumor boards are a standard part of medical care in the U.S. and are generally made up of several different types of doctors, including surgeons and radiation oncologists, who review patients' cases and make recommendations for their treatment.
The study's authors write in the Journal of the National Cancer Institute that previous research found hospitals dedicate about 50 hours per month of their doctors' time to participation in tumor boards.
"It is interesting that despite the fact that tumor boards seem like a good thing and they are so well established, there is so little literature on them," said Keating.
She and her colleagues wanted to know whether tumor boards actually made a difference.
To do that, they linked together information and records from the VA's 138 medical centers on cancer patients treated between 2001 and 2004.
The team found that 75 percent of the medical centers had at least one tumor board that discussed most of the conditions the researchers were interested in: colorectal, lung, prostate, breast and blood cancers.
Then, using established national guidelines, the researchers developed a list of 27 markers for the quality and type of care patients received.
For example, the researchers checked whether patients with stage II or III rectal cancer got the recommended dose of chemotherapy and radiation before surgery to remove the cancer.
Overall, the researchers found the presence of a tumor board was only linked with differences in seven of the 27 markers.
"And some of those seven were actually a situation where the tumor board was associated with worse care," Keating said.
In addition, recommended care for specific types of malignancies, such as blood cell cancers, was more often seen in centers with no tumor board (56 percent) or only a general tumor board (61 percent) than in centers with tumor boards specializing in blood cancers (39 percent).
"This is a little bit off-putting because it challenges the conventional wisdom that tumor boards are a good thing," said Dr. Douglas Blayney, a professor of medicine at the Stanford School of Medicine in California.
"I think the main issue that remains to be answered: Did the recommendations of the tumor boards actually get carried out?" added Blayney, who wrote an editorial accompanying the study.
"We think patients benefit from having their cases reviewed at the outset, but we leave it to the medical system to get acted upon," he said.
Keating said researchers need to do a "deep dive" into tumor boards to find out more. She said they still need to know how the tumor boards are structured, and what types of cases are discussed.
Until then, "I do think that people and centers who are investing time and energy in their tumor boards should really think about how they are structured, and if they're set up in a way to improve patient care," she said.
Blayney told Reuters Health that he doesn't think hospitals should scrap their tumor boards based on these findings, because there are new possibilities with new technology.
"The promise of telemedicine technology makes extra use of academic centers available to patients who may live in rural locations and doctors who are remote from the academic centers," he said.
For example, the rural doctors of a woman with breast cancer can conference with a tumor board that specializes in breast cancer at a large, urban academic center.
"Again it's tapping into the knowledge and experience of a broad range of physicians," Blayney said.

(Reuters) - A Christmas Eve glitch traced to Amazon.com Inc that shuttered Netflix for users from Canada to South America highlights the risks that companies take when they move their datacenter operations to the cloud.
While the high-profile failure - at least the third this year - may cause some Amazon Web Services customers to consider alternatives, it is unlikely to severely hurt a fast-growing business for the cloud-computing pioneer that got into the sector in 2006 and has historically experienced few outages.
"The benefits still outweigh the risks," said Global Equities Research analyst Trip Chowdhry.
"When it comes to the cloud, Amazon has got it right."
The latest service failure comes at a critical time for Amazon, which is betting that AWS can become a significant profit generator even if the economy continues to stagnate. Moreover, it is increasingly targeting larger corporate clients that have traditionally shied away from moving critical applications onto AWS.
AWS, which Amazon started more than six years ago, provides data storage, computing power and other technology services from remote locations that group thousands of servers across areas than can span whole football fields. Their early investment made it a pioneer in what is now known as cloud computing.
Executives said last month at an Amazon conference in Las Vegas they could envision the division, which lists Pinterest, Shazam and Spotify among its fast-growing clients, becoming its biggest business, outpacing even its online retail juggernaut. Evercore analyst Ken Sena expects AWS revenue to jump 45 percent a year, from about $2 billion this year to $20 billion in 2018.
The service has boomed because it is cheap, relatively easy to use, and can be shut off, scaled back or ramped up quickly depending on companies' needs. As the longest-running player in the game, Amazon now boasts the widest array of datacenter products and services, plus a broader stable of clients than rivals like Google Inc, Rackspace Inc and Salesforce.com Inc.
Outages such as the one that took down Netflix and other websites on the eve of one of the biggest U.S. holidays are part and parcel of the nascent business, analysts say. Moreover, outages have been a problem long before the age of cloud computing, with glitches within corporate datacenters and telecommunications hubs triggering myriad service disruptions.
COMING SOON: POST-MORTEM
Amazon's latest service failure comes months after two high-profile outages that hit Netflix and other popular websites such as photo-sharing service Instagram and Pinterest. Industry executives, however, say its downtimes tend to attract more attention because of its outsized market footprint.
Netflix - which CEO Reed Hastings said relies on AWS for 95 percent of its datacenter needs - would not comment on whether they were pondering alternatives. Analysts say the video streaming giant is unlikely to try a large-scale switch, partly because all cloud providers experience outages.
"Despite a steady stream of these service outages, the demand for cloud services offered by AWS, Google, etc. continues to escalate because these services are still reliable enough to satisfy customer expectations," said Jeff Kaplan, managing director of consultancy ThinkStrategies Inc.
"They offer cost-savings and elasticities that are too attractive for companies to ignore."
But "Netflix and other organizations which rely on AWS will have to reexamine how they configure their services and allocate their service requirements across multiple providers to mitigate over-dependency and risks."
AWS spokeswoman Rena Lunak said the outage was traced to a problem affecting customers at its oldest data center, run out of northern Virginia, which was linked also to the June failure.
The latest glitch involved a service known as Elastic Load Balancing, which automatically allocates incoming Web traffic across multiple servers in order to boost the performance of a website. She declined to provide further details about the outage, saying the company would be publishing a full post-mortem within days.
AWS has traditionally been used by start-up tech companies and smaller businesses that anticipate rapid growth in online traffic but are unwilling or unable to shell out on IT equipment and management upfront.
The company has more recently started winning more and more business from larger corporations. It has also set up a unit that caters to government agencies.
Regardless, Amazon's clientele would do well not to put all their eggs in one basket, analysts say.
"Service outages do occur, but they are not common enough to cause users of these services to abandon today's Cloud service providers at significant rates. In fact, every major Cloud service provider has experienced outages," Kaplan said.
"Therefore, organizations that rely on these services are putting backup and recovery systems and protocols in place to mitigate the risks of future outages."

BERLIN ? As a capital city, Berlin has endured more than its fair share of division over the years.?Now new battle lines are being drawn over what some see as a fight for the city's character.
The conflict began when City Hall announced?its intention to phase out the vast majority of Berlin's historic gas lamps?as?part of an ambitious project to make the city carbon-neutral by 2050.
With nearly 43,000 gas-powered streetlights, Berlin has more than any other city in the world. In fact, more than one in six in the city are gas.
Some date back to the 19th century; others were erected immediately after World War II as the occupying Soviet forces made restoring light to the devastated city a priority.
In recent years, guided tours have been run to picturesque areas, with sightseers attracted by the?distinctive warm, yellowish glow of gas lamps.

Pollution, expense
Think Beacon Hill in Boston or San Diego's Gaslamp Quarter ? but on a much larger scale ? and cue the outrage.
But with annual running costs for fuel and maintenance as much as $700 for some lamp models, and carbon dioxide emissions almost ten times that of an equivalent electric light, there are now strong financial and environmental incentives to replace gas with electric alternatives.
The city's current modernization program?(link in German) will see 8,000 highway lamps, mostly dating from the early 1950s, replaced with new electric lights.
City authorities say the figures speak for themselves.
?The energy used by those 8,000 gas lamps could power 100,000 electric lights.?And replacing them would cut energy costs by 90 percent, reduce carbon dioxide emissions by 9,200 tons per year and save a chunk of the $1.6 million spent each year just on replacement gas mantles.

Petra Rohland, spokeswoman for Berlin's Department of Urban Development, said the current refit would be complete by end of 2016 ? and would recoup the cost within nine years.
All but a few of the city's gas-powered lamps will eventually go.
"Five percent of the historic gas lights, especially the candelabra, will be kept as originals in the future," Rohland said.
'Knock down the Brandenburg Gate'
It's a future that fills some Berliners like Paul Harrison with dread.
Harrison is a member of a growing band of preservation societies?who oppose the wholesale replacement of gas lights.
He challenges the environmental and financial arguments put forward by the city to justify the changes.

"If we're just talking about saving money, we could knock down the Brandenburg Gate," he said ironically. "After all, that costs a lot to keep going, to keep clean."
Harrison's group, Gaslight Culture, is calling for the dismantling to be suspended - and for talks between all interested parties.

"We haven't started to explore the possibilities, such as different forms of financing or even sponsorship of streets or districts," he said.
Harrison deplored what he described as ?the rejection of a working system.??And the replacement LEDs would be ?prohibitively expensive? and ?far from convincing? as alternatives.

Such rejection of new technology would be a disappointment to Andre Braun, who has spent years developing LED illumination that?mimics the color of gas light?(in German).
For Braun, whose workshop is on the same site as the former Berlin gas plant where his father once worked, the search for the perfect replacement is nothing short of a crusade.

The way he talks about working with gas is reminiscent of how a fisherman might talk of the sea.
"It's so very difficult to work with," Braun said. "The extremes of temperature make it a constant battle ... unlike electricity, which is a dead light, gas gives a living light. But that's tough to recreate in an LED."
"Some people think I'm crazy to spend all this time trying to replicate the look of the gas lamp," he said. "But they are beautiful; gas lights have no glare, you can look right into them."

The storm is forecast to hit the Northeast sometime Wednesday evening, bringing a mix of heavy sleet, snow or heavy rain, depending on its track.?
Motorists were warned to expect whiteout conditions in falling and blowing snow overnight Wednesday in the Buffalo area, where some locations could get as much as 14 inches by late Thursday, NBC station WGRZ of Buffalo reported.

So if you?re having trouble keeping all the scenarios straight, we?re here to help.
Below we list every team that?s still in playoff contention, where they stand in the playoff order, and how their paths to the playoffs look over the final two weeks of the season.
AFC
Already clinched playoff berths
1. Texans: Clinch home-field advantage throughout the playoffs if they win one more game.
2. Broncos: Clinch a first-round playoff bye if they win out.
3. Patriots: Clinch the No. 3 seed if they win out, need help to get a first-round playoff bye.
4. Ravens: Clinch the AFC North with one more win.
Control their own destiny
5. Colts: Clinch a wild card with one more win.
6. Bengals: Clinch a playoff berth with a win over the Steelers on Sunday.
7. Steelers: Clinch a playoff berth with two more wins.
Need a lot of help
8. Dolphins: Can reach the playoffs if they beat the Bills and Patriots, and the Steelers beat the Bengals but lose to the Browns, and the Bengals lose to the Ravens in addition to losing to the Steelers, and the Jets lose at least one more game.
NFC
Already clinched playoff berths
1. Falcons: Clinch home-field advantage throughout the NFC playoffs with one more win.
2. 49ers: Clinch a first-round playoff bye with two more wins.
3. Packers: Clinch the No. 3 seed if they win out, need help to get a first-round playoff bye.
Control their own destiny
4. Redskins: Clinch the NFC East if they win out.
5. Seahawks: Clinch a wild card if they win one more game.
6. Cowboys: Currently behind the Vikings in the NFC, but clinch the NFC East if they win out.
7. Giants: Currently behind the Vikings in the NFC, but clinch a playoff berth if they win out.
Need some help
8. Vikings: Would be a wild card if the playoffs started today, but even if they win out they need either the Giants to lose once or the Seahawks to lose twice in order to make the playoffs.
9. Bears: Can reach the playoffs by winning out and having both the Giants and Vikings lose once, or either the Giants or Vikings lose once plus the Seahawks losing twice, or a couple of more complex scenarios. Basically, the Bears lose most tiebreakers and therefore will likely need to finish 10-6 while the other NFC wild card hopefuls finish 9-7.
Need a lot of help
10. Rams: Technically still alive but would need to win both remaining games while the Vikings, Bears and Giants all lose out, plus either the Redskins or Cowboys lose in Week 16 and then the team that loses in Week 16 wins the Week 17 Cowboys-Redskins game.
And there you have it. A Rams-Dolphins Super Bowl is still possible.
